What to bring
- Photo ID
- Current medication list, including doses
- Relevant medical records, if available
- Insurance information, if applicable
- Recent lab results or imaging
- Completed new patient forms, or arrive early to complete them
Your first visit
Your first appointment includes a health review, a focused exam and a discussion of your health goals with your doctor.
Cooper Medical primarily operates on a self-pay basis. New patient visits start at $200.
